Who is on a Hospice Team?

Our experienced team will provide care that respects the dignity and preferences of the patient while supporting the emotional and practical needs of their loved ones.  Your team will consist of:

• Physicians address health issues related to the end of life including minimizing uncomfortable symptoms.
• Nurses (RN) manage comfort and care while providing guidance.
• Nursing Aides (CNA) assist with activities of daily living such as bathing and dressing.
• Social Workers (LMSW) offer support with end-of-life preparation, legal paperwork and funeral arrangements.
• Spiritual Support and Chaplains provide spiritual support through visits and can connect you with community resources.
• Bereavement Coordinators provide support while preparing for the loss of a loved one and are available as a resource for up to one year after the death.
